Guidance Notes

Who can apply?

We welcome applications from or on behalf of children and young people up to their 25th birthday, who need specialist mobility equipment or a wheelchair to help them become more independent. The child or young person must be resident in the UK.

What you can apply for

We provide funding for mobility equipment, including:

  • Powered wheelchairs
  • Manual wheelchairs
  • Sports, all terrain, off road and beach wheelchairs
  • Specialist buggies
  • Walkers
  • Specialist Trikes
  • Mobile hoists
  • Power packs (to add power to a manual chair)

Car seats and harnesses.

  • Note: if you have a registered Motability car, we advise that you apply to Motability for the car seat. For more information, see the link here: Motability. Motability has told us that the easiest way to start the process with them is by calling 0344 8000 900 and selecting option 2. This will put you directly through to a Grants Case Manager who will take you through their application process.

We do not provide funding for:

  • Static equipment such as specialist chairs, seating and beds
  • Standing frames
  • Static exercise bikes and treadmills
  • Hoists (except mobile hoists)
  • Vehicles or vehicle adaptations
  • Ramps, lifts and building works
  • Sheds for storing mobility equipment
  • Non-mobility equipment such as communication aids, lap tops or sensory rooms
  • Wheelchairs that are mainly for school use (for example, to traverse a large secondary school between lessons). We believe these should be funded by a young person’s Education, Health and Care Plan
  • Equipment for a school or community group such as a specialist trike

This list is not exhaustive and if you are unsure please phone our Mobility Services Team on [email protected] or 020 8347 8111.

How to apply

Please read this information carefully before completing your form. If you are filling the application form out on a computer, please save it first.

If you are a parent or carer please try (where possible) to complete this application form together with the child or young person who will be using the equipment, particularly when answering questions about their life experiences or interests.

This application form is an agreement between the recipient’s family and my AFK and by signing it you are agreeing to abide by our Terms and Conditions. It is important, therefore, that you read them carefully before signing and returning your application.

Step 1

Before applying to us you will need to arrange an assessment for the applicant with a representative of the equipment supplier(s) and an occupational therapist or physiotherapist. Assessments can usually take place at home or in school or college.

For equipment costing more than £1,500 we usually ask that the applicant has two assessments and sends us two quotations for different brands of equipment, that offer similar functions. This is regardless of whether you are contributing to the cost of the equipment.

The supplier will provide you with a price quotation for the equipment you want and the physiotherapist or occupational therapist should provide you with a letter setting out why your child needs this specific brand and model of equipment and the functions and benefits it offers.

*my AFK usually funds the less expensive of the two pieces of equipment, unless the therapist confirms that it does not meet the applicants’ needs.

Step 2

Return the completed application form together with the quotation/s and the therapist’s supporting letter. It can often be helpful to us to have a photograph of the applicant, for example to help with fundraising appeals. Please enclose a photograph if you feel able to.

Our waiting list for equipment can be up to nine months or sometimes a little longer. We do hope to shorten the time, particularly for items under £1000. A wait for a powerchair can be up to eighteen months or even a little longer.
